Clinical Nurse Educator

The Clinical Nurse Educator serves in a patient support role by working with the intake department to facilitate timely and efficient discharges through the proper vetting of a patient’s suitability for home infusion and/or respiratory care and providing necessary education to allow timely/efficient discharges to occur. The Clinical Nurse Educator does this in collaboration with physician offices, hospital case managers, discharge planners and social work staff at area hospitals and Skilled Nursing Facilities. This person serves as a patient care advocate and facilitates the transition to post-hospitalization/facility care.

The Clinical Nurse Educator will travel as needed to various facilities as needed as patients are identified as potential home infusion and/or respiratory patients.

Minimum Qualifications:

• Current NY State RN licensure, with a minimum of three years homecare or clinical experience
• Excellent oral and written communication skills; ability to make group presentations
• Knowledge of home medical and infusion services preferred
• Personable, organized, and self-directed
• Ability to prioritize and multi-task
• Computer literacy
• Valid driver’s license and access to reliable transportation required

Work Environment:

• Travel in the community at large
• Hazards include risk of exposure to blood borne and airborne pathogens
• Possible exposure to traffic hazards and poor road conditions

Special Physical Demands:

• Manual dexterity necessary to manipulate sharps regularly without injury
• Requires ability to move about (standing, sitting, operating a motor vehicle, etc.)
• Clear speech, adequate hearing and near visual acuity are essential for effective performance
• Must possess ability to write and/or input data on a keyboard
• Must maintain a pleasant, social, positive, and outgoing attitude
