Upstate Homecare is seeking a motivated and compassionate Enteral Sales Representative to drive growth in our enteral nutrition therapy services. In this dynamic, field-based role, you will build strong relationships with hospital case managers, discharge planners, social workers, and staff at Skilled Nursing Facilities to generate patient referrals and support a smooth transition from facility to home care.
As a key liaison between our internal Enteral Intake team and external healthcare partners, you will serve as a patient advocate, ensuring coordinated, timely, and effective care delivery in alignment with Upstate Homecare’s mission and high clinical standards.
Identify and cultivate referral sources within hospitals, SNFs, and other healthcare settings.
Collaborate closely with internal intake and clinical teams to support patient transitions and care planning.
Educate healthcare professionals on Upstate Homecare’s enteral services and coordinate care solutions tailored to each patient.
Act as a patient care advocate and resource for families and healthcare providers.
Maintain regular, professional communication with referral partners and internal departments.
Track referral activity and maintain accurate records for performance and growth analysis.
Prior sales or marketing experience preferred, especially in healthcare or medical services.
Excellent verbal and written communication skills; comfortable presenting to individuals and groups.
Strong organizational skills with the ability to prioritize, multitask, and work independently.
Proficient with computers and common business software.
Knowledge of home medical equipment (HME) or infusion reimbursement is a plus.
Must possess a valid driver’s license and have reliable transportation.
Community-based role with frequent travel to hospitals, SNFs, and healthcare facilities.
Exposure to variable weather conditions and typical road travel risks.
