As an RN Clinical Liaison, you'll be the bridge between hospital-based healthcare professionals and our home infusion services. Your primary responsibility will be to generate referrals for acute infusion therapies (e.g., TPN, antibiotics, inotropes, and nutrition support) by collaborating with hospital case managers, discharge planners, and social workers. You’ll also coordinate with internal departments such as pharmacy, intake, nursing, and operations to ensure a seamless transition from hospital to home care.
This is a field-based position focused on developing strategic relationships with hospitals, Skilled Nursing Facilities (SNFs), and other care partners to promote our services and uphold our mission of patient-first care.
Cultivate and maintain strong relationships with physicians, hospitals, SNFs, ALFs, home health agencies, and payors.
Collaborate with case managers, physicians, and patients to coordinate smooth hospital discharges.
Educate patients and caregivers on infusion therapy and equipment during initial home setup.
Serve as a knowledgeable resource and patient advocate, ensuring consistent communication between stakeholders.
Support internal teams (clinical, operations, reimbursement, and management) to deliver outstanding care and service.
Stay informed on local market trends, competitors, and payer requirements.
Meet or exceed sales and referral growth targets within the assigned territory.
Provide responsive sales support and identify new business opportunities.
Registered Nurse (RN) – Required
Minimum 1 year of nursing experience – Required
BLS Certification – Preferred
Bachelor’s degree in healthcare, business, or a related field (or equivalent experience)
3+ years of home care or clinical experience with a proven performance record
Sales or marketing experience preferred
Familiarity with home infusion and DME reimbursement practices
Valid driver’s license and reliable transportation – Required
Field-based role with frequent travel throughout the community
Exposure to healthcare environments and potential biohazards
Some travel-related risks (e.g., weather, road conditions)
